APPLICATION PROCESS

The Joe Hammond Award application process aims to identify and honor individuals who exemplify Joe Hammond’s commitment to community, mentorship, and philanthropy in the West End of Louisville. The process involves several steps to ensure a thorough and fair evaluation of all nominees.

Application Steps

  1. Nomination Form Submission

    • Deadline: All nominations must be submitted by August 31, 2024.

    • Form Access: Nomination forms can be accessed online at [Award Nomination Form URL] or obtained from the Joe Hammond Foundation office.

    • Details Required: The form must include:

      • Nominee’s full name, contact information, and affiliation.

      • Nominator’s full name, contact information, and relationship to the nominee.

      • A detailed description of the nominee’s contributions to the community, including specific examples of community involvement, leadership, mentorship, philanthropic activities, and partnerships.

      • A summary of the nominee’s impact on the West End community, highlighting how their efforts align with Joe Hammond’s legacy.
         

  2. Supporting Documents

    • Required Documentation:

      • At least two letters of recommendation from individuals who can attest to the nominee’s contributions and impact.

      • Testimonials from community members or organizations that have benefited from the nominee’s work.

      • Any relevant media coverage, project reports, or other documentation that supports the nomination.

    • Submission Method: Supporting documents can be uploaded online with the nomination form or mailed to the Joe Hammond Foundation office.
       

  3. Review and Evaluation

    • Initial Screening: The Joe Hammond Foundation staff will conduct an initial screening of all applications to ensure they meet the eligibility criteria.

    • Review Panel: A diverse panel of community leaders, past award recipients, and representatives from the Joe Hammond Foundation will review and evaluate the applications based on the selection criteria:

      • Impact on the community

      • Sustainability of efforts

      • Innovation and initiative

      • Alignment with Joe Hammond’s legacy

    • Scoring: Each application will be scored based on a standardized rubric that reflects these criteria.
       

  4. Interviews (if applicable)

    • The review panel may conduct interviews with top candidates or their nominators to gain further insights into their contributions and impact.

    • Interviews will be scheduled during September 2024 and can be conducted in person or virtually.
       

  5. Final Selection

    • The review panel will convene to discuss and finalize the selection of the award recipient.

    • The recipient will be notified by October 1, 2024.
       

  6. Award Presentation

    • The Joe Hammond Award will be formally presented at The S4S Gala on October 19, 2024.

    • The recipient will be honored during the gala, and their contributions to the West End community will be celebrated.
